Cleft Lips and Cleft Palates – How Are They Treated?
For parents who find out that their child will be born with a cleft lip, cleft palate, or both, there are usually many questions about what this birth defect will mean for the life of their child, and how the condition can be treated. Cleft lips and cleft palates are not only physically disfiguring, but they can dramatically affect a baby’s ability to feed, breath, hear, and eventually learn to speak. For these reasons, it is very important that these babies have access to care and treatment plans.
Treatment plans need to be very specific to each child and circumstance. Usually a team of doctors is needed to care for the infant, and the child often needs follow-up care even into the pre-teen and teen years.
The structural defects of the upper lip and palate lead to difficulties being able to breastfeed or bottle feed. A baby struggles because he or she cannot easily suck at the nipple, a response that is typically natural. Because of this, most babies born with a cleft lip and/or cleft palate need at least one surgery to correct the condition. Often multiple surgeries are needed to not only correct the structures, but to address physical healing and minimize scarring.
Before some surgeries, doctors sometimes need to perform stretching procedures such as nasoalveolar molding. This prepares the surrounding tissue for the necessary repairs. A palatoplasy, a procedure to fix the cleft palate, typically needs to be done by the time the baby is 18 months old for maximum benefits. Cleft lips can be surgically fixed in the first year of life. If feeding problems are significant, alternative feeding solutions may be needed until the series of surgeries are complete.
When the gums and mandible are involved in the birth defect, some children grow up to need extensive orthodontic care. Surgery of the jaw and even within the gum area is sometimes needed before the child reaches puberty in order to help align the bite and correct any over/under bites.
Parents should also be sure that a full team of care includes audiologists and nutritionists, especially if the birth defects are significant. Nutritionists can help ensure that the growing baby is getting enough nutrients and help parents adjust the dietary needs as warranted. Cleft palates can interfere with normal hearing structures and not let fluid drain from the middle ear. This leads to hearing loss and eventual speech disorders. Speech therapy may also be needed if the problems are severe or not 100% correctable by surgery.
